ask your physician to make a referral to us. Or, if you are
being discharged from a health care facility (hospital,
rehabilitation unit or nursing home), speak to the social
worker/case manager there about a referral to the Visiting
Nurse Association of Greater Lowell.

These are some of the
questions we will ask:
* Name and address of
patient
(to determine if the patient is living in our service area)
* Condition of
patient/your concern
(to help us determine if our services are appropriate to
meet the patients needs)
* Name of
physician
(the physician must order services and approve the plan for
home care)
* Insurance
coverage
(so that we can inform the patient of how and to whom costs
will be billed)
Every year
we admit nearly 5,000 patients of all ages. Generally, VNA
home health care is appropriate for patients who:
* have recently been
discharged from a medical facility and still need
professional health care
* have been diagnosed
with a condition that requires teaching and information
about medications, coping and lifestyle
adjustments
* have a medical need
for short-term nursing or rehabilitation services at home
until the condition stabilizes
With the
information provided, we will coordinate with the patient,
the physician, the case manager, insurance company, and
appropriate caregivers to develop a comprehensive treatment
plan.
In most
cases, home care services can begin within 24 to 48 hours of
a completed referral.

community resources.
